Can I use my membership benefits immediately?
Yes! Once you enroll, your cleanings, exams, and X-rays are ready to schedule right away.
Does the membership plan cover emergencies?
Your exams and necessary X-rays are covered. Treatment for emergency issues receives your member discount.
Can I combine membership savings with other insurance?
Our membership is designed for those without insurance. It cannot be combined with other plans or coverage.

Membership Duration:
Your membership lasts for one full year from the date signed. Services must be used within that term and do not roll over. Membership is non-transferable.
Family/Dual Plan Start Date:
All covered members under a family or dual plan share the same start and end date. If one member enrolls today, the term for all members begins today and ends one year from the enrollment date. Additional members cannot be added mid-cycle with a separate start or end date.
Payment Options:
Annual plans are paid in full at the time of enrollment and renew every 12 months.
Monthly plans require a 12-month commitment and will be automatically charged to your card on file each month.
Card Authorization:
By enrolling in a monthly plan, you authorize Dahle Family Dental to securely store your card information and charge the monthly fee on or around the same date each month.
Failed Payments:
If a monthly payment fails, you agree that you will be responsible for either:
– The remaining balance of the 12-month plan, or
– The full retail value of services already used —
Whichever amount is greater.
If payment cannot be collected, the balance may be referred to a third-party collections agency.
Account Standing:
Membership benefits may be suspended if your account is not in good standing.
Cancellation:
You may cancel your membership at any time by submitting a written request to dahle@dahlefamilydental.com. If services have already been used, any unpaid portion of your agreement will still be due. No refunds will be provided for unused benefits or partial months. Your plan auto-renews unless canceled at least 15 days before the renewal date.
Scheduling & Attendance:
Appointments must be canceled or rescheduled at least 24 business hours in advance. Missed appointments or short-notice cancellations may result in the loss of that benefit (e.g., a cleaning). A pattern of missed appointments may lead to removal from the plan without refund.
Limitations:
– This is not dental insurance.
– Membership benefits are valid only at Dahle Family Dental.
– Discounts do not apply to referred services or third-party lab costs.
– Discounts are based on Dahle Family Dental’s Usual and Customary Rates, which are subject to change.
– "Household members" are defined as spouses, domestic partners, or children under 26 living at the same address.
